Services
CMS has many programs to help provide care outside of your office to your patients. When we analyze your patient population, we identify which patients can qualify for which programs. As we care for patients, we inherently fulfill the requirements for many of these programs. Our monthly report will contain every CPT code completed for each patient for you to accurately bill for. For every service we complete, we charge you a flat rate that accounts for all of the work our care managers put into helping your patients.
